ABSTRACT

INTRODUÇÃO: A análise da implicação na pesquisa se opõe a uma suposta neutralidade resultante do afastamento do objeto, porque o/a pesquisador/a coloca a sua subjetividade em análise, o contexto histórico-social da pesquisa, assim como, a intersubjetividade de todos os envolvidos nela. A implicação integra o processo de construção do conhecimento na busca de maior compreensão dos fenômenos, sendo um dos conceitos fundamentais da psicossociologia. OBJETIVO: O presente estudo objetiva apresentar um relato de experiência sobre a análise da implicação em uma pesquisa clínico-qualitativa respaldada teórico-metodologicamente pela psicossociologia francesa e psicodinâmica do trabalho. Essa investigação ocorreu entre 2018 e 2022, tendo se dado parcialmente no período da pandemia da COVID-19. METODOLOGIA: Empregaram-se os seguintes dispositivos como método de análise da implicação: os diários de campo, as supervisões, as reuniões de equipe, a participação em um grupo de convivência e a psicoterapia individual. Assim, a pesquisadora procurou abordar os seus afetos e sentimentos envolvidos ao longo das etapas da pesquisa. RESULTADOS E DISCUSSÃO: Foi possível, pois, evidenciar a implicação como fonte, paradoxalmente, de processos criativos e, em alguns casos, também de alienação. CONCLUSÃO: Apesar da identificação de alguns destes aspectos negativos da implicação da pesquisadora, se pode ampliar o conhecimento sobre o objeto da pesquisa.


INTRODUCTION: The implication analysis is opposed to a supposed neutrality because the researcher analyzes his own subjectivity, his historical and social context, as well as the intersubjectivity of the actors involved in a research. Implication analysis is one of the fundamental concepts of psychosociology. It enables one to understand phenomena and builds knowledge. OBJECTIVE: The present study aims to present an experience report about an implication analysis of a clinical-qualitative research that was based on French psychosociology and psychodynamics of work theory and methodology. This investigation happened from 2018 to 2022, partially carried out during the COVID-19 pandemic. METHOD: There were used the following resources to access the implication analysis: dailies, supervisions, team meetings, participation in a reflection group, and individual psychotherapy. Thus, the researcher related feelings and affections involved in each one of the research stages. RESULTS AND DISCUSSION: This study highlighted the implication as a source that paradoxically allows creative processes but also, in some cases, alienation. CONCLUSION: Despite some negative aspects of the implication researcher, it was possible to expand the knowledge about the research object.


INTRODUCCIÓN: El análisis de la implicación en la investigación se opone a una supuesta neutralidad resultante de la distancia del objeto porque el investigador analiza su propia subjetividad, el contexto histórico-social de la investigación, así como la intersubjetividad de todos los involucrados en la investigación. La implicación forma parte del proceso de construcción del conocimiento en la búsqueda de una mejor comprensión de los fenómenos, siendo uno de los conceptos fundamentales de la psicosociología. OBJETIVO: Este estudio tiene como objetivo presentar un relato de experiencia sobre el análisis de la implicación en una investigación clínico-cualitativa sustentada teórica y metodológicamente por la psicossociologia francesa y la psicodinámica del trabajo. Esta investigación se llevó a cabo entre 2018 y 2022, habiéndose realizado parcialmente durante el período de la pandemia de COVID-19. MÉTODO: Se utilizaron como método de análisis de la implicación los siguientes dispositivos: diarios de campo, supervisiones, reuniones de equipo, participación en un grupo de convivencia y psicoterapia individual. Así, la investigadora buscó abordar sus afectos y sentimientos involucrados a lo largo de las etapas de la investigación. RESULTADOS Y DISCUSIÓN: Este estudio destacó la implicación como fuente, paradójicamente, de procesos creativos y, en algunos casos, alienación. CONCLUSIÓN: A pesar de la identificación de algunos de estos aspectos negativos de la implicación de la investigadora, se logró ampliar el conocimiento sobre el objeto de investigación.

ABSTRACT

ABSTRACT BACKGROUND: Precisely determining the aspects related to an instrument's validity and reliability measures allows for greater assurance of the quality of the results. OBJECTIVES: To analyze the psychometric properties of The Providers Survey in the Brazilian context of mental health services. DESIGN AND SETTING: The instrument validation study was conducted in Montes Claros, Minas Gerais, Brazil. METHODS: The validation study was conducted using the Consensus-based Standards for the Selection of Health Measurement Instruments checklist to analyze its validity and reliability. RESULTS: A committee of expert judges performed content validation after which the Content Validity Index was calculated. Construct validation took place through Exploratory Factor Analysis using the Kaiser-Meyer-Olkin Test criterion and Bartlett's Sphericity Test. Reliability was verified using test-retest reliability. The significance level adopted for the statistical tests was 5% (P < 0.05). The final instrument comprised 54 questions. The Content Validity Index was 97%. Exploratory Factor Analysis identified a Kaiser-Meyer-Olkin index of 0.901 and Bartlett's Sphericity Test with P < 0.001. We obtained a Cronbach's alpha coefficient of 0.95 and an intraclass correlation coefficient of 0.849. CONCLUSIONS: The Providers Survey, translated and adapted into Portuguese, was named the Work Assessment Instrument for the Recovery of Mental Health. It presented adequate psychometric properties for evaluating work-related practices for the recovery of psychosocial care network users.

ABSTRACT

Actualmente, en Chile se ha evidenciado un aumento de la población vegetariana, sin embargo, existen escasos datos sociodemográficos, por lo cual, el objetivo del presente estudio es caracterizar, según antecedentes sociodemográficos y alimentarios a personas vegetarianas chilenas en el año 2022. Se utilizó el paradigma cuantitativo y el diseño metodológico fue no experimental, descriptivo, exploratorio de corte transversal. Se aplicó una encuesta de caracterización mediante Google Forms a 504 personas y para el análisis de la información se utilizó el programa SPSS v. 24. El 85,7% declaró ser de género femenino, la edad promedio fue 26,3 ± 5,7 años, 45,5% de los encuestados se declararon ovolactovegetarianos, seguido por vegetarianos estrictos (38,7%) y la principal motivación fue principios animalistas (68,7%). El 87,8% cursó educación superior y el 36,5% correspondió al área de salud. El principal motivo de asistencia al nutricionista fue el asesoramiento con un 80%. Se destacó que la población vegetariana consideró que es fundamental la asistencia a un profesional nutricionista para practicar este estilo de vida de forma óptima, por lo que es fundamental que los profesionales de salud se involucren en conocer a esta población, con el objetivo de otorgar una atención más cercana e integral. Finalmente, la presente investigación corresponde al primer estudio que permite caracterizar a la población vegetariana en Chile.


Currently, in Chile there has been an increase in the vegetarian population; however, there are few sociodemographic data, therefore, the objective of this study is to characterise, according to sociodemographic and dietary background, Chilean vegetarians in the year 2022. The quantitative paradigm was used and the methodological design was non-experimental, descriptive, exploratory cross-section. A characterisation survey was applied using Google Forms to 504 people and SPSS v. 24. 85.7% declared to be female, the average age was 26.3 ± 5.7 years, 45.5% of those surveyed declared themselves lacto-ovo vegetarians, followed by strict vegetarians (38.7%) and the main motivation was animalistic principles (68.7%). 87.8% attended higher education and 36.5% corresponded to the health area. The main reason for helping the nutritionist was the advice with 80%. It was highlighted that the vegetarian population considered that it is essential to assist a professional nutritionist in practising this lifestyle optimally, so it is essential that health professionals get involved in getting to know this population, with the aim of providing closer and more comprehensive care. Finally, this research corresponds to the first study that allows characterising the vegetarian population in Chile.

ABSTRACT

Resumo Durante a pandemia de COVID-19, a saúde mental dos profissionais de saúde, que trabalharam diretamente nos serviços voltados para o cuidado dos pacientes afetados pela doença, tornou-se questão fundamental a ser considerada, dado os diversos desdobramentos que essa atuação gerou para esses profissionais. O objetivo deste artigo foi compreender desafios e demandas dos profissionais de saúde em termos de suporte para lidar com o desgaste emocional e físico com a atuação na chamada linha de frente durante a pandemia de COVID-19. A abordagem metodológica qualitativa se deu a partir de entrevistas semiestruturadas realizadas em ambiente online com esses profissionais, passados os primeiros meses de pandemia. O lugar de herói em que eles foram colocados, ainda que apenas nos discursos midiáticos, logo deu espaço para que as fragilidades destes e das relações de trabalho aparecessem: estresse, medo e o desejo de escuta e acolhimento. A teoria da dádiva de Marcel Mauss foi trazida considerando que novas formas de leitura e interpretação das relações de trabalho em saúde contribuem para reformulações necessárias e urgentes do contexto em que se encontram hoje, visando a saúde mental e, mais amplamente, a saúde integral dos profissionais da área de saúde.


Abstract The mental health of health professionals who worked directly in services during the COVID-19 pandemic to care for patients affected by the disease became a fundamental issue to be considered, given the several consequences of this activity for these professionals. This article aimed to understand the challenges and demands of health professionals concerning support to address the emotional and physical exhaustion of working on the so-called frontline during the COVID-19 pandemic. The qualitative methodological approach was based on semi-structured interviews conducted online with these professionals after the first months of the pandemic. The hero's place in which they were set, even if only in media discourses, soon gave way to their weaknesses and fragile work relationships to emerge: stress, fear, and the listening and reception desire. Marcel Mauss' gift theory was brought up considering that new ways of reading and interpreting health work relationships contribute to necessary and urgent reformulations of their current context, targeting mental health and, more broadly, the comprehensive health of healthcare professionals.

ABSTRACT

Resumo O artigo versa sobre o mundo do trabalho da saúde, especialmente no SUS no contexto da pandemia no Brasil. O artigo utilizou dados das pesquisas "Condições de trabalho dos profissionais de saúde no contexto da COVID-19 no Brasil" e "Os trabalhadores invisíveis da saúde: condições de trabalho e saúde mental no contexto da COVID-19 no Brasil". A análise dos dados comprova que a pandemia evidenciou problemas estruturais existentes no âmbito do Sistema Único de Saúde, envolvendo a gestão da FTS o que pode ser interpretado como mais um dos reflexos das desigualdades socioeconômicas já existentes no país. Destacam-se: a reduzida oferta de educação permanente, a regulação do cuidado híbrido, precarização, desproteção no ambiente de trabalho, frágil biossegurança levando a trágicas taxas de adoecimento e mortes de trabalhadores da saúde. Conclui mostrando a importância de formulação de políticas públicas no âmbito da gestão da educação e do trabalho no SUS que assegurem a discussão sobre cuidado híbrido como nova forma de atuar sem perder qualidade, a necessidade de se rever questões referentes a: educação permanente, proteção, valorização e redução das desigualdades apontadas entre os contingentes profissionais analisados nesse artigo.


Abstract This article addresses the world of healthcare work, especially in the Brazilian Unified Healthcare System (SUS) in the context of the COVID-19 pandemic in Brazil. This study used data from the following surveys: "Working conditions of health professionals in the context of COVID-19 in Brazil" and "The Invisible health workers: working conditions and mental health in the context of COVID-19 in Brazil". Data analysis proves that the pandemic highlighted existing structural problems within SUS, involving the issue of healthcare workforce (HWF) management, which can be interpreted as another reflection of the socioeconomic inequalities that already exist in the country. This article highlights: the reduced provision of permanent education, the regulation of hybrid care, precariousness, a lack of protection in the work environment, as well as fragile biosecurity leading to tragic rates of illness and death of health workers. Our study concludes by showing the importance of formulating public policies in the scope of education and work management in SUS that ensure the discussion on hybrid care as a new way of acting without losing quality, together with the need to review issues related to permanent education, protection, valuation, and reduction of inequalities pointed out among the professional contingents analyzed in this article.

ABSTRACT

Resumo O artigo analisou o processo de precarização das relações de trabalho no setor da saúde, demonstrando como essa situação gerou sofrimento dos trabalhadores. O texto buscou registrar como a adoção institucional das concepções econômicas neoliberais, de redução das políticas sociais de Estado, acopladas a um histórico de rejeição cultural aos direitos trabalhistas, proporcionou a atração dos mecanismos de precarização do trabalho, notadamente, a terceirização da mão de obra e da prestação de serviços. No segundo momento, o texto aponta as impropriedades jurídicas cometidas, apresentando as consequências da fragilização do vínculo jurídico nas condições de trabalho desses trabalhadores, principalmente no que se refere às jornadas de trabalho e a remuneração, destacando-se, também, o quanto a disseminação desses vínculos representou um fator de rebaixamento geral dos salários e direitos trabalhistas e previdenciários no setor. Foram apresentados, ainda, dados das pesquisas da Fiocruz sobre condições de trabalho durante a pandemia. Conclui mostrando a urgência de se resgatarem os institutos jurídicos trabalhistas que foram abandonados no percurso histórico estudado, para direcionar a novo rumo as políticas públicas na saúde.


Abstract The present article analyzed the process of precarious work relations in the health sector, demonstrating how this situation generated illness and suffering among workers. The text sought to register how the institutional adoption of neoliberal economic conceptions, of the reduction of State social policies, coupled with a history of cultural rejection of labor rights, provided the attraction of mechanisms of precarious work for the public sector, notably, the outsourcing of labor and the provision of services. In a second moment, the text points out the legal improprieties committed, presenting the consequences of the weakening of the legal bond in the working conditions of these workers, mainly regarding working hours and remuneration, also highlighting how much the dissemination of these links represented a factor of a general reduction in wages and labor and social security rights in the sector. Data from Fiocruz surveys on working conditions during the pandemic were also presented. The article concludes by showing the urgency of rescuing the legal labor institutes that were abandoned in the historical path studied in order to guide the public health policies in a new direction.

As práticas de trabalho em saúde têm historicamente passado por transformações principalmente a partir dos processos de formação dos profissionais da saúde. O profissional sanitarista, ou bacharel em saúde coletiva vem somando esforços e ocupando espaços de trabalho multiprofissionais fortalecendo a perspectiva transversal do cuidado em saúde em diversos campos de atuação. Objetivo:Este estudo objetiva relatar a experiência de construção do processo de trabalho de um bacharel em Saúde Coletiva na Atenção Primária à Saúde do Campo em meio a pandemia de Covid-19. Metodologia:O período de abrangência foi de 2020 a 2022, ocorrendo no município de Caruaru, Pernambuco. A atuação do bacharel em Saúde Coletiva se deu na perspectiva do Núcleo de Apoio àsaúde da famíliae atenção básica,atuando junto a duas equipes de saúde da família de territórios rurais. Resultados: O processo de trabalho se desenvolveu em três principais frentes, sendo elas: 1) Atenção à saúde com base territorial; 2) Articulação com redes intra e intersetoriais; 3) Reorganização do trabalho multiprofissional na Atenção Primária à Saúde. Conclusões: Sendo assim, é possível constatar que o processo de trabalho do bacharel em Saúde Coletiva está em constante movimento, delimitando as suas possibilidades de atuação, bem como os seus limites quanto a um fazer ainda desconhecido em realidades como a Atenção Primária. O avanço da discussão ocorre em paralelo a formação e inserção destes profissionais nos mais diversos contextos a exemplo da saúde do campo. Nota-se a necessidade de desenvolver novos estudos e experiências formativas de bacharéise saúde coletiva conectados aos territórios do campo, apostando na capilarização e ampliação da presença deste profissional nas realidades singulares do Sistema Único de Saúde (AU).


Health work practices have historically undergone transformations, mainly from the training processes of health professionals. The sanitary professional, or bachelor in public health, has been joining efforts and occupying multidisciplinary workspaces, strengthening the transversal perspective of health care in different fields. Objective:This study aims to report the experience of building the work process of a Bachelor of Public Health in Primary Health Care in the Countryside in the midst of the Covid-19 pandemic. Methodology: The coverage period was from 2020 to 2022, taking place in the municipality of Caruaru, Pernambuco. The performance of the bachelor in Collective Health took place from the perspective of the Support Center for family health and primary care, working with two family health teams from rural territories. Results:The work process was developed on three main fronts, namely: 1) Health care based on territory; 2) Articulation with intra and intersectoral networks; 3) Reorganization of multidisciplinary work in Primary Health Care.Conclusions:Therefore, it is possible to verify that the work process of the bachelor in Collective Health is in constant movement, delimiting their possibilities of action, as well as their limits regarding an action still unknown in realities such as Primary attention The advancement of the discussion occurs in parallel with the training and insertion of these professionals in the most diverse contexts, such as rural health. There is a need to develop new studies and training experiences for bachelors and collective health connected to rural territories, focusing on the capillarization and expansion of the presence of this professional in the unique realities of the Unified Health System (SUS) (AU).


Las prácticas del trabajo en salud históricamente han sufrido transformaciones, principalmente a partir de los procesos de formación de los profesionales de la salud. El profesional sanitario, o licenciado en salud pública, viene sumando esfuerzos y ocupando espacios de trabajo multidisciplinarios, fortaleciendo la perspectiva transversal del cuidado de la salud en diferentes campos. Objetivo: Este estudio tiene como objetivo relatar la experiencia de construcción del proceso de trabajo de un Licenciado en Salud Pública en la Atención Primaria de Salud en el Campo en medio de la pandemia de la Covid-19. Metodología: El período de cobertura fue de 2020 a 2022, ocurriendo en el municipio de Caruaru, Pernambuco. La actuación de la licenciatura en Salud Colectiva se dio en la perspectiva del Centro de Apoyo a la salud de lafamilia y atención primaria, trabajando con dos equipos de salud de la familia de territorios rurales. Resultados: El proceso de trabajo se desarrolló en tres frentes principales, a saber: 1) Atención de la salud con base en el territorio; 2) Articulación con redes intra e intersectoriales; 3) Reorganización del trabajo multidisciplinario en la Atención Primaria de Salud. Conclusiones: Por tanto, es posible constatar que el proceso de trabajo del licenciado en Salud Colectiva está en constante movimiento, delimitando sus posibilidades de actuación, así como sus límites frente a un actuar aún desconocido en realidades como la Atención primaria. El avance de la discusión ocurre en paralelo con la formación e inserción de estos profesionales en los más diversos contextos, como la salud rural. Existe la necesidad de desarrollar nuevos estudios y experiencias de formación para licenciaturas y salud colectiva vinculadas a los territorios rurales, con foco en la capilarización y ampliación de la presencia de ese profesional en las realidades singulares del Sistema Único de Salud (SUS) (AU).

Resumen Objetivos: La investigación se propuso tres objetivos: describir la representación social de salud y bienestar; caracterizar la labor de los profesionales de salud para promover el bienestar; establecer similitudes y diferencias entre lo que conocen y realizan, respecto del concepto de la Organización Mundial de la Salud. Materiales y métodos: Es un estudio descriptivo, transversal, multicéntrico (Cuba, México, Panamá y Perú); después de una revisión bibliográfica, se llevó cabo una encuesta y consulta a expertos. Se utilizó un muestreo no probabilístico y se realizaron análisis de estadística descriptiva. Resultados: Los profesionales de salud, perciben el bienestar muy agradable y cómodo, posible, dulce, bueno, responsable, flexible, seguro, blanco, algo ligero, pero un poco corto y difícil; a la salud la perciben muy agradable, posible, cómoda, flexible, dulce, responsable, segura, buena, blanca, ligera y larga pero difícil; la mayoría cree conocer los conceptos salud y bienestar de OMS/OPS, pero al compararlos con los establecidos por estos organismos, no son correctos, ni completos y su conocimiento resulta insuficiente. Conclusiones: Los profesionales de salud no tienen una comprensión total de la relación entre ambos conceptos; la mayoría realiza las acciones para promover el bienestar que le resultan fáciles; no se perciben como entes activos y transformadores en las actividades generadoras de una vida saludable y, por tanto, no se encuentran en consonancia con las transformaciones necesarias que privilegian salud y bienestar.


Abstract Objectives: The research proposed three objectives: to describe the social representation of health and well-being; characterize the work of health professionals to promote well-being; establish similarities and differences between what they know and do, with respect to the concept of the World Health Organization. Materials and methods: It is a descriptive, cross-sectional, multicenter study (Cuba, Mexico, Panama and Peru); after a literature review, a survey and expert consultation were carried out. Non-probabilistic sampling was used and descriptive statistical analyzes were performed. Results: The health professionals, perceive well-being as very pleasant and comfortable, possible, sweet, good, responsible, flexible, safe, white, somewhat light, but a bit short and difficult; health is perceived as very pleasant, possible, comfortable, flexible, sweet, responsible, safe, good, white, light and long but difficult; most believe they know the concepts of health and well-being of WHO/PAHO, but when compared to those established by these organizations, they are not correct or complete, and their knowledge is insufficient. Conclusions: In conclusion, they do not have a full understanding of the relationship between both concepts; most take actions to promote well-being that are easy for them; they are not perceived as active and transforming entities in the activities that generate a healthy life and therefore, they are not in line with the necessary transformations that privilege health and well-being.

ABSTRACT

Abstract Developing effective learning strategies to strengthen mental health professionals' capacities and deliver evidence-based interventions in their communities is urgent. We developed and evaluated an online training program for the Intervention Guide for Mental, Neurological and Substance Use Disorders in Non-specialized Health Settings. Nine hundred and seventy-five health professionals in Mexico were enrolled in the training program, during the period of social distancing brought about by the COVID-19 pandemic. Participants completed a pre-post online evaluation strategy including Knowledge screening, assessment of Learning Activities, and performance in Programmed-Simulated cases to evaluate knowledge and skills for the assessment, management, and follow-up of Mental, Neurological and Substance Use Disorders. We found that participants improved their knowledge and skills from training on the mhGAP online course. Notably we observed these positive results regardless of sex, profession, institution, or social vulnerability rating of participants, suggesting that this is a relevant training program for primary care staff. These results contribute to the Mental Health Gap Action Programme and advance the use of online teaching and evaluation technologies in this field.


Resumen El desarrollo de estrategias efectivas de aprendizaje para fortalecer las competencias de los profesionales de la salud mental y brindar intervenciones basadas en evidencia en sus comunidades es necesario. El objetivo del presente trabajo fue desarrollar y evaluar un programa de entrenamiento en línea para la Guía de Intervención en Trastornos Mentales, Neurológicos y por Uso de Sustancias en nivel de atención de salud no especializada. Participaron 975 profesionales de la salud mexicanos durante el período de distanciamiento social provocado por la pandemia de COVID-19. Los participantes completaron una evaluación previa y posterior que incluyó un cuestionario de conocimientos, actividades de aprendizaje y la ejecución en casos simulados programados para evaluar el conocimiento y las habilidades para la evaluación, el manejo y el seguimiento de los trastornos mentales, neurológicos y por uso de sustancias. Los resultados indicaron que los participantes mejoraron sus conocimientos y habilidades en función de su participación en el curso en línea, independientemente del sexo, la profesión, la institución o la vulnerabilidad social de los participantes, sugiriendo que se trata de un programa de formación relevante para el personal de atención primaria. Resumen El presente estudio tuvo por objetivo analizar las características de artículos científicos publicados durante el último quindenio respecto a la atención en salud a personas trans en América Latina y el Caribe. Se desarrolló una revisión sistematizada de artículos publicados en cuatro bases de datos, entre los años 2005 y 2020, en la que se encontraron 20 estudios que fueron objeto de análisis. En su mayoría, estos dan cuenta de las percepciones que tienen las comunidades trans y, en menor frecuencia, las experiencias de los profesionales de la salud sobre la atención en salud. Los resultados permiten identificar barreras que influyen en los procesos de atención y los desafíos para mejorarla. Sobresale la formación deficiente e insuficiente en los profesionales sobre identidades trans y los enfoques diferenciales y afirmativos del género, y la necesidad de fortalecer los abordajes integrales entre las comunidades académicas, de la salud y la generación de políticas públicas transformativas.


Abstract Health care is important for the transitions of people with trans life experiences and to strengthen the processes of health and disease. Health professionals are educating and accompanying agents; however, the literature frequently reports that there are barriers perceived by professionals regarding their competencies to accompany such processes, in addition to others related to social representations regarding trans identities, the limited scientific evidence regarding the processes of gender reaffirmation and representing the communities themselves according to their historical and sociocultural characteristics. The aim of this study was to analyze the characteristics of scientific articles published during 15 years regarding health care for trans persons in Latin America and the Caribbean. A systematized review of articles published in four databases between 2005 and 2020 was carried out. 20 studies were analyzed in the research. Most of them report the perceptions of trans communities and, less frequently, the experiences of health professionals on health care. The results allow the identification of barriers that influence the processes of care and the challenges to improve it. The deficient and insufficient training of professionals on trans identities and differential and affirmative gender approaches stand out, as well as the need to strengthen comprehensive approaches among the academic and health communities and the generation of trans affirmative public policies. On the other hand, the results evidence a construction of science with respect to gender that is cisnormative and invisibilizes gender diversities. In addition, many of the studies establish vertical relationships between participants, including researchers, which generates research dynamics in which trans communities are considered more as informants and less as expert peers, community researchers or participants in a collaborative and co-creative research process. These findings support the need and renew the urgency to generate processes of knowledge construction with and for trans communities that allow nurturing educational, social, public policy and health spaces that represent their needs for gender transitions, in case the person considers so, and that welcome in an integral and humanized way the processes of health and disease. Research on health and health care in trans communities both in Colombia and in the rest of Latin America and the Caribbean does not show the implementation of health policies focused on the community and that respond to their health needs. Health professionals, around the world, should accompany from care actions focused on the processes of self-identification and self-determination of gender. Being recognized and made visible based on gender identities and expressions has a positive impact on the mental health of trans people and their well-being. In professional practice, it is important and essential to overcome the barriers of social inequality, promoting a critical social vision and understanding of the reality of this population. It is necessary to expand research from gender diversities and separate from the LGBTIQ+ umbrella given the complex social, cultural, biological, and psychological diversities faced by each community. As for health professionals, it is necessary to deepen the measurement and understanding of attitudes, knowledge, beliefs, skills and gaps identified by all staff, from doctors and nurses to institution managers. This in order to articulate and triangulate information and develop intervention programs aimed at comprehensive and accessible care, especially for their reports about the little information on sexual and gender diversity received during their professional training process and organizational culture. Finally, each country should develop clinical guidelines based on local, national, and international evidence that responds to the psychological and sociocultural needs and experiences of trans communities that include the process of gender transitions from hormone therapy, affirmative surgeries and other non-binary processes of gender expressions, family, psychosocial support, and community accompaniment.

INTRODUÇÃO: Com a pandemia da COVID-19, os profissionais de saúde tiveram que adotar estratégias de enfrentamento (coping) para lidar com o stress acarretado pela pandemia. OBJETIVO: avaliar as estratégias de coping utilizadas pelos profissionais da saúde (médicos, dentistas, enfermeiros, técnicos em enfermagem) das 52 unidades de Saúde da Família do município de Piracicaba no início da pandemia. METODOLOGIA: Este é um estudo observacional de corte transversal de cunho quantitativo, em que foram utilizados 2 questionários autoaplicáveis: 1) sobre os dados sociodemográficos constituído de 8 questões abertas: idade, profissão, tempo no serviço público, escolaridade, grupo de risco, sexo, estado civil e número de filhos e 2) o Inventário de Coping de Folkman e Lázarus (1985) contendo 66 questões representando ações ou pensamentos específicos utilizados em uma situação de estresse. RESULTADOS: Participaram desta pesquisa 198 profissionais da saúde, após assinatura do termo de consentimento livre e esclarecido, em que 90,9% da amostra era do gênero feminino; 26,8% estavam no grupo de risco; 49% possuíam até o segundo grau de escolaridade. A categoria profissional predominante do estudo foi a dos agentes de saúde com 49,2%, seguido dos auxiliares de enfermagem (16%), enfermeiros (14%), médicos (8,3%), dentistas (6,8%), auxiliares de saúde bucal (4,7%) e outro (1%). A estratégia de coping mais utilizada foi a reavaliação positiva (0,749), seguida da resolução de problemas (0,692). Quanto ao grupo de risco, a fuga-esquiva foi a forma de enfrentamento mais utilizada (0,806). DISCUSSÃO: Houve poucos estudos referentes ao estresse em profissionais da saúde da assistência primária até o momento. Este estudo, como outros no contexto da pandemia de 2019, mostrou que a reavaliação positiva, a resolução de problemas e a busca de apoio social foram as estratégias mais utilizadas pelos profissionais da saúde e as mulheres foram as mais afetadas por serem maioria atuantes na área da saúde. CONCLUSÃO: verificou-se que os profissionais da saúde utilizaram principalmente a reavaliação positiva e a resolução de problemas como formas de enfrentamento e que são necessárias medidas que ofereçam melhores condições de trabalho, treinamentos e valorização desses profissionais.


INTRODUCTION: With the COVID-19 pandemic, health professionals had to adopt coping strategies to deal with the stress caused by the pandemic. OBJECTIVE: to evaluate the coping strategies used by health professionals (doctors, dentists, nurses, nursing technicians) from the 52 Family Health units in the city of Piracicaba at the beginning of the pandemic. METHODOLOGY: This is a crosssectional observational study of a quantitative nature, in which 2 self-administered questionnaires were used: 1) on sociodemographic data consisting of 8 open questions: age, profession, time in public service, education, risk group, sex, marital status and number of children and 2) the Coping Inventory by Folkman and Lázarus (1985) containing 66 questions representing specific actions or thoughts used in a stressful situation. RESULTS: 198 health professionals participated in this research, after signing the free and informed consent form, in which 90.9% of the sample was female; 26.8% were in the risk group; 49% had up to the second level of schooling. The predominant professional category in the study was health agents with 49.2%, followed by nursing assistants (16%), nurses (14%), physicians (8.3%), dentists (6.8%), assistants oral health (4.7%) and another (1%). The most used coping strategy was positive reappraisal (0.749), followed by problem solving (0.692). As for the risk group, escape-avoidance was the most used form of coping (0.806). DISCUSSION: There have been few studies regarding stress in primary care health professionals to date. This study, like others in the context of the 2019 pandemic, showed that positive reassessment, problem solving and the search for social support were the strategies most used by health professionals and women were the most affected because they are the majority active in the area of health. CONCLUSION: It was found that health professionals mainly used positive reassessment and problem solving as ways of coping and that measures are needed to offer better working conditions, training and appreciation of these professionals

OBJETIVO: Evaluar las estrategias de enfrentamiento utilizadas por los profesionales de la salud (médicos, odontólogos, enfermeros, técnicos de enfermería) de las 52 unidades de Salud de la Familia del municipio de Piracicaba al inicio de la pandemia. METODOLOGÍA: Se trata de un estudio observacional transversal de carácter cuantitativo, en el que se utilizaron 2 cuestionarios autoadministrados: 1) sobre datos sociodemográficos constituidos por 8 preguntas abiertas: edad, profesión, tiempo en el servicio público, escolaridad, grupo de riesgo, sexo, estado civil y número de hijos y 2) el Inventario de Afrontamiento de Folkman y Lázarus (1985) que contiene 66 preguntas que representan acciones o pensamientos específicos utilizados en una situación estresante. RESULTADOS: 198 profesionales de la salud participaron de esta investigación, previa firma del formulario de consentimiento libre e informado, en el que el 90,9% de la muestra era del sexo femenino; el 26,8% estaban en el grupo de riesgo; el 49% tenía hasta el segundo nivel de escolaridad. La categoría profesional predominante en el estudio fue la de agentes de salud con 49,2%, seguida de auxiliares de enfermería (16%), enfermeros (14%), médicos (8,3%), odontólogos (6,8%), auxiliares de salud bucal (4,7%) y otro (1%). La estrategia de afrontamiento más utilizada fue la reevaluación positiva (0,749), seguida de la resolución de problemas (0,692). En cuanto al grupo de riesgo, el escape-evitación fue la forma de afrontamiento más utilizada (0,806). DISCUSIÓN: Hasta la fecha existen pocos estudios sobre el estrés en profesionales de salud de Atención Primaria. Este estudio, como otros en el contexto de la pandemia de 2019, mostró que la reevaluación positiva, la resolución de problemas y la búsqueda de apoyo social fueron las estrategias más utilizadas por los profesionales de la salud y las mujeres fueron las más afectadas por ser las mayoritarias activas en el área de salud. CONCLUSIÓN: Se constató que los profesionales de la salud utilizan principalmente la reevaluación positiva y la resolución de problemas como formas de enfrentamiento y que se necesitan medidas para ofrecer mejores condiciones de trabajo, capacitación y valorización de estos profesionales.

ABSTRACT

Objective: This systematic review addressed the effects of the competency level of medical students, physicians, and practitioners in nutrition education for optimum patient care. It also addresses the perceived inadequate didactic contact hours of nutrition education. Method: There were 55 studies selected, and 25 were used for this review including quantitative and qualitative studies. The data were divided into four groups: quantitative data, qualitative data, reports, and news articles. The categorization of the literature was as follows: 17 quantitative, 2 qualitative, 4 news articles, 1 manual, and 1 symposium report. These were evaluated to produce a credible qualitative meta-analysis of available data. Data Sources: The systematic review used databases and citation indexes including Embosses, PubMed, JAMA Network, Medline, Elsevier, and Oxford Academics, these include journals, reports of Symposiums, and news articles to ascertain evidence-based data Results: The findings of this review revealed the significant effect inadequate contact nutrition education has on the competency level of medical students, physicians, and practitioners. Limitations: Limitations of this review include several external factors. Although universities are expected to implement approximately 44 hours of nutrition education for the competency level of medical students and physicians during their tenure, this may not be the case at all schools. Many offer only 15-25 didactics hours. In addition, in some cases, the comparisons are not equal but the researcher was unable to establish the hours and a basic curriculum structure.

Objective:To analyze the importance of post competency elements of community public health professionals, and to provide data and method support for the construction and cultivation of community public health professionals.Methods:In September to October 2020, 25 experts were selected in the study, and the interval value hesitant fuzzy entropy method was used to analyze the importance of post competency elements. Excel 2013 and SPSS 25.0 were used to calculate the data.Results:Among the primary index of post competency elements, the order of importance were professional quality (0.207), professional knowledge and skills (0.204), integrative thinking (0.200), personal characteristics (0.199), basic professional ability (0.191); for second-level indicators, for second-level indicators,critical thinking (0.105), systematic thinking (0.095), public health knowledge and skills (0.044) were the top three of post competency elements were the top three of post competency elements.Conclusion:The recessive professional accomplishment dominated by professional ethics, the integrative thinking of public health dominated by critical thinking, and the professional knowledge and skills dominated by public health knowledge and skills should be focused on.

ABSTRACT The genus Bartonella encompasses 38 validated species of Gram-negative, facultative intracellular bacteria that colonize the endothelial cells and erythrocytes of a wide spectrum of mammals. To date, 12 Bartonella species have been recorded infecting humans, causing diseases of long historical characterization, such as cat scratch fever and trench fever, and emerging bartonellosis that mainly affect animal health professionals. For this reason, this study aimed to report a documented case of Bartonella bovis infecting a veterinarian from Mexico by the amplification, sequencing and phylogenetic reconstruction of the citrate synthase (gltA) and the RNA polymerase beta-subunit (rpoB) genes, and to report the natural course of this infection. To our knowledge, this work is the first to report the transmission of B. bovis via needlestick transmission to animal health workers in Latin America.

Resumo Este texto propõe desenhar modelos de cuidado mais efetivos e adequados às características da população idosa. Para esse intuito, dispõe acerca da necessidade de ênfase nas instâncias leves de cuidado, ou seja, foco na coordenação, na prevenção e no monitoramento do cliente, de forma a minimizar desperdícios, oferecendo uma assistência de melhor qualidade e a custos reduzidos. Outrossim, são apresentados os instrumentos de avaliação epidemiológica utilizados e o passo a passo de todos os profissionais da equipe de saúde. Desta forma, pretende-se viabilizar a melhor assistência para o público idoso, torná-la sustentável e transformar não apenas o cuidado voltado para esse segmento, mas o sistema de saúde como um todo.


Abstract This text proposes the design of care models that are more effective and appropriate for the characteristics of the aged population. To this end, the article expounds on the need for emphasis on low-complexity care that focuses on coordination, prevention and client monitoring to reduce wastefulness, offer better quality care and lower costs. Furthermore, the epidemiological assessment instruments used and the detailed routine of all health professionals are presented. Thus, it is endeavoured to deliver the best care for the aged and, within an estimated timeframe of around 20 years, render this sustainable, transforming not only care for this segment of users, but for the health system as a whole.

Resumo A saúde planetária vem se consolidando como área transdisciplinar do conhecimento, fundamentada na caracterização e mitigação dos impactos antropogênicos sobre a saúde do ser humano e dos sistemas terrestres. O estilo de vida em sociedades capitalistas impulsiona a degradação ambiental e suas consequências (como emergência climática e perda de biodiversidade), relacionando-se também à alta prevalência de doenças crônicas não transmissíveis. No entanto, essas questões não costumam receber a atenção necessária no processo de ensino e aprendizagem dos profissionais de saúde. Assim, este ensaio objetiva contribuir para sistematização das informações que correlacionam os fatores ambientais e a saúde humana na perspectiva do pensamento sistêmico, destacando a importância de se pensar a saúde sistêmica no âmbito das profissões da saúde. Observa-se que o profissional de saúde, sendo ponte entre o conhecimento científico e a sociedade, pode desempenhar os papéis de mediador do cuidado integral e curador do conhecimento. Além disso, é preciso incentivar o pensamento sistêmico no processo formativo em saúde para que esses papéis sejam alcançados, tanto em atitudes individuais quanto coletivas.


Abstract Planetary Health has been consolidated as a transdisciplinary area of knowledge grounded in characterizing and mitigating anthropogenic impacts on the health of human beings and earth systems. The capitalist lifestyle drives environmental degradation and its consequences (such as climate emergency and biodiversity loss), and is related to the high prevalence of chronic non- communicable diseases. This information, however, usually lacks the necessary emphasis in health teaching and learning process. Thus, this essay aims to systematize information that correlates environmental factors and human health from a systemic thinking perspective, highlighting the importance of reflecting on systemic health within health professions. As the bridge between scientific knowledge and society, health professionals can play the role of integral care agent and knowledge curator. Moreover, we must encourage systemic thinking in health education to achieve these roles, both in individual and collective attitudes.

Resumo Reconhece-se a escassez de investigação sobre o mercado de trabalho do setor da saúde. Com a ampliação dos fenómenos de precarização objetiva e subjetiva, decorrente da agenda neoliberal de mercadorização do valor do trabalho e das reformas gestionárias da saúde, a "atipicidade" dos vínculos laborais e a "insegurança" de vida dos profissionais de saúde têm introduzido lógicas de segmentação e polarização do mercado de trabalho. Este deixa de ser palco apenas de mercados protegidos, passando a incluir mercados desregulados e "híbridos". Neste artigo pretende-se explorar os sentidos, com implicações desiguais, da (des)regulação do mercado de trabalho no setor da saúde em Portugal. Parte-se de fontes secundárias e de investigação qualitativa suportada em 32 entrevistas semiestruturadas feitas com profissionais de saúde, que atestam o fenômeno da crescente individualização e subcontratação laboral. Propõe-se um modelo conceitual que capte os sentidos de (des)regulação, num continuum de regulação social e mobilidade profissional, traduzindo mercados protegidos (profissionais e internos) e mercados híbridos (terciarizados e secundários) a serem testados por confronto empírico e investigação futura.


Abstract The need for further research on the labor market in the health sector is acknowledged. With the expansion of the phenomena of objective and subjective precariousness, resulting from the neoliberal agenda of commodification of the value of work and the managerial reforms in health, the "atypicality" of labor ties and the "insecurity" of health professionals' lives have introduced segmentation and polarization logics into the labor market. The latter is no longer the stage for protected markets, but includes deregulated and "hybrid" markets. This article aims to explore the directions, with unequal implications, of the (de)regulation of the labor market in the health sector in Portugal. We begin from secondary and qualitative research supported by 32 semi-structured interviews with health professionals, which attest to the phenomenon of increasing individualization and labor subcontracting. A conceptual model is proposed that captures the meanings of (de)regulation, in a continuum of social regulation and professional mobility, translating protected markets (professional and internal) and hybrid markets (outsourced and secondary) to be tested by empirical confrontation and future research.

ABSTRACT Objective: to understand the potentialities and challenges for the care provided by professionals in the primary health care context. Method: a qualitative research study of the participatory action type, based on Paulo Freire's Research Itinerary, which consists of Thematic Investigation; Coding and Decoding; and Critical Unveiling. It was developed in the first half of 2021, with the participation of 20 professionals, including physicians, nurses, dentists, nursing technicians, dental assistants and community agents, all from a Basic Health Unit in Santa Catarina. Results: the health professionals revealed the team's engagement as potentialities, as well as welcoming, which strengthens interpersonal relationships in Primary Care. As challenges, they highlighted the limited professional appreciation and scarce material and human resources, factors that generate tensions for the assistance to be provided. Conclusion: engagement and interpersonal relationships reveal the professionals' commitment and dedication to promoting comprehensive and good quality care, seeking to overcome the limitations inherent to assistance in Primary Health Care.


RESUMEN Objetivo: comprender las potencialidades y los desafíos para la asistencia desarrollada por los profesionales en el contexto de la Atención Primaria de la Salud. Método: investigación cualitativa y del tipo acción participante, fundamentada en el Itinerario de Investigación de Paulo Freire, que consiste en Investigación Temática; Codificación y Decodificación; y Revelación Crítica. Se desarrolló en el primer semestre de 2021 con la participación de 20 profesionales, entre médicos, enfermeros, odontólogos, técnicos de Enfermería, auxiliares de consultorio dental y agentes comunitarios, todos de una Unidad Básica de Salud de Santa Catarina. Resultados: los profesionales de la salud indicaron lo siguiente como potencialidades: compromiso del equipo, al igual que la recepción, que fortalece las relaciones interpersonales en la Atención Primaria. Como desafíos, destacaron la limitada valorización profesional y escasos recursos materiales y humanos, factores que generan tensiones sobre la asistencia. Conclusión: el compromiso y las relaciones interpersonales revelan la dedicación de los profesionales por promover una atención integral y de buena calidad, procurando suplir las limitaciones de la asistencia provista en la Atención Primaria de la Salud.

Método: pesquisa qualitativa, do tipo ação participante, fundamentada no Itinerário de Pesquisa de Paulo Freire, que consiste de Investigação Temática; Codificação e Descodificação; e, Desvelamento Crítico. Foi desenvolvida no primeiro semestre de 2021, com a participação de 20 profissionais, entre médico, enfermeiro, odontólogo, técnico de enfermagem, auxiliar de consultório dentário e agente comunitário, todos de uma Unidade Básica de Saúde de Santa Catarina. Resultados: os profissionais de saúde desvelaram como potencialidades o engajamento da equipe, assim como o acolhimento que fortalece as relações interpessoais na atenção primária. Como desafios ressaltaram a limitada valorização profissional e parcos recursos materiais e humanos, fatores esses que geram tensionamentos para a assistência. Conclusão: o engajamento e a relação interpessoal revelam o comprometimento e dedicação dos profissionais em promover uma atenção integral e de qualidade, buscando suprir as limitações da assistência na atenção primária à saúde.

ABSTRACT

Este estudo traz dados de uma pesquisa de doutorado que objetivou conhecer a relação entre a precarização do trabalho e a saúde dos profissionais de um Centro de Atenção Psicossocial (CAPS). Trata-se de uma pesquisa de cunho qualitativo no campo da Saúde do Trabalhador. Foram realizadas entrevistas individuais semiestruturadas e observações sistemáticas com os trabalhadores. A partir dos depoimentos e das observações, foram percebidos problemas de ordem de gestão e de recursos materiais e humanos necessários para o bom funcionamento do serviço que geraram preocupação e sofrimento aos profissionais. Um outro componente que também colaborou para a precarização do trabalho desses profissionais foi o trabalho terceirizado. Além disso, os profissionais expressaram suas experiências e seu cotidiano de trabalho por meio de sofrimento, angústia e dificuldades para realização do trabalho. O sofrimento do trabalhador da saúde mental também tem suas particularidades, inerentes ao próprio tipo de trabalho, visto que tais profissionais estão expostos ao sofrimento e à dor de outras pessoas


This study brings data from a doctoral research which aimed to understand the relationship between the precarization of the work and the health of professionals from Psychosocial Care Center (CAPS). The study is qualitative research in the field of Occupational Health. Semi-structured individual interviews were carried out as well as systematic observation of the workers. Based on the interviewees' testimonies and analyzes, some management problems as well as lack of material and human resources necessary for the correct functioning of the service were noticed, which caused concern and suffering on the workers. Another component that also contributed to the precarious work of these professionals was outsourced work. Besides, workers expressed their experiences and their daily work through suffering, anguish, and difficulties in performing their work correctly. The suffering of mental health professionals also has its particularities, inherent to the type of work itself, since they are constantly exposed to other people's suffering and pain

ABSTRACT

Objective The first wave of COVID-19 was challenging for healthcare workers. This study analyzed the ways of coping with stress at a university hospital. Method A Sociodemographic Characterization, Risk and Exposure Assessment, Burnout Assessment Tool (BAT-23), and COVID-19 Coping Scale were responded online by 181 professionals. Results The sample was composed mainly of women, white, married, physicians, with one job. Over 80% of the sample were at high exposure and risk for infection, with 17.1% having tested positive. The most reported stressors were risks of transmitting the disease, being hospitalized, and being separated from loved ones; 11% presented the risk of/probable burnout, with exhaustion and emotional impairment. They presented adaptive coping strategies, such as problem-solving and information-seeking, with a negative correlation between adaptive coping and burnout. Being a physician with maladaptive coping, in psychiatric care, and having religious beliefs were predictors of burnout. Conclusion Promoting adaptive coping may improve the mental health of these workers.


Objetivo A primeira onda da COVID-19 foi desafiadora para os profissionais de saúde. Este estudo analisou o enfrentamento do estresse em um hospital universitário. Método De forma online, 181 profissionais responderam: Caracterização Sociodemográfica, Avaliação do Risco e Exposição, Burnout Assessment Tool (BAT-23) e Escala de Coping da COVID-19. Resultados A amostra foi composta principalmente por mulheres, caucasianos, casados, médicos, com um emprego. Tiveram alta exposição e risco à infecção mais de 80% da amostra, com 17,1% positivos. Os estressores mais relatados foram: riscos de transmitir a doença, ser hospitalizado e separado de pessoas queridas; 11% apresentaram risco/provável de burnout, com exaustão e prejuízo emocional. Apresentaram estratégias mais adaptativas, como Resolução de Problemas e Busca de Informações, havendo uma correlação negativa entre coping adaptativo e burnout. Ser médico, com coping mal adaptativo, em atendimento psiquiátrico e ter crença religiosa foram preditores de burnout. Conclusão Promover um coping adaptativo poderá melhorar a saúde mental desses trabalhadores.
